ABSTRACT

Objective To explore the clinical effect and safety of Suhexiang Pills () in the treatment of patients with tachycardia after severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection. Methods A total of 138 patients with tachycardia after SARS-CoV-2 infection admitted to eight hospitals such as 971st Hospital of the PLA Navy, Changzhou Second People's Hospital, Xuzhou First People's Hospital, Henan Provincial People's Hospital, Henan Chest Hospital from February 2023 to March 2023 were randomly divided into control group and treatment group, with 87 patients in the treatment group and 51 in the control group. Patients in the control group were po administered with betaloc, once a day, and the initial dose was 23.75 mg, adjusted in time according to the patient's heart rate. Patients in the treatment group were po administered with Suhexiang Pills, 1 pill/time, twice daily. Patients in two groups were treated for 7 d. The clinical efficacy of the two groups was observed, and the heart rate and cardiac function indexes, RR interval, blood oxygen saturation and adverse reactions were compared between the two groups before and after treatment. Results After treatment, the total effective rate of the treatment group was 98.85%, and the total effective rate of the control group was 90.20%, and the difference between the two groups was statistically significant (P < 0.05). After treatment, heart rates were significantly decreased in both groups (P < 0.05), and the heart rates of the treatment group were significantly better than those of the control group (P < 0.05) on the 7th day of treatment. After treatment, the level of left ventricular ejection fraction (LVEF) in both groups was significantly higher than that before treatment (P < 0.05), and there was statistical difference between the treatment group and the control group (P < 0.05). The levels of left ventricular end diastolic dimension (LVEDD) and left ventricular end-systolic diameter (LVESD) in the treatment group significantly decreased than that before treatment (P < 0.05), and there was no statistical difference compared with the control group (P > 0.05). After treatment, the maximum RR interval in both groups reached the normal range on the third day, and the treatment group was significantly better than the control group (P < 0.05). Blood oxygen saturation of the treatment group was significantly increased on the 7th day of treatment compared with before treatment (P < 0.05), but there was no statistical significance between the two groups (P > 0.05). There was no significant difference in the total incidence of adverse events between the two groups (P > 0.05). Conclusion Suhexiang Pills decrease heart rates in patients with tachycardia after SARS-CoV-2 infection, which was equivalent to the effect of western medicine, and can protect heart, improve heart function to a certain extent.Copyright © 2023 Editorial Office of Chinese Traditional and Herbal Drugs. ABSTRACT

Background: The immune system is highly susceptible to changes of zinc levels and this might imply a different response against infection. Prior evidence suggests some benefit on viral infection prognosis after zinc supplementation. We aim to study the efficacy of zinc supplementation in SARS-CoV-2 infection outcomes. Method(s): This is an unicenter prospective, randomized cliinical trial where unvaccinated individuals with moderate SARS-CoV-2 infection without endorgan failure were randomized to standard of care+oral zinc for 15 days (three times per day a tablet of 83mg of Zn acetate equals to 75 mg of Zn element) (zSoC) (n =37) or standard of care alone (SoC) (n = 34). The primary combined outcome was death due to SARS-CoV-2 or intensive care unit (ICU) admission. Secondary outcomes included length of hospital stay (LoS) and time to clinical stability (defined as: oxygen saturation >94% [FiO2 21%], normalized level of consciousness [baseline], HR < 100rpm, systolic BP >90mm Hg,Temperature < 37.2degreeC). Wilcoxon-Mann-Whitney test generalized Odds ratio (ORs) and 95% confidence intervals (CIs) for differences in outcomes between SoC and zSoC. A logistic regression model was fitted adjusted by age, sex, severity and comorbidity to compare the primary outcome between SoC and zSoC. Result(s): Seventy-one participants were recruited. No significant differences in terms of age, gender and comorbidities nor in SoC were found between groups (Table 1). 14-day Mortality was 2.90 % (2 participants) in the SoC group and none in zSoC. ICU admission rates were, respectively, 8 (23%) and 1 (2.7%) (OR: .098;95% CI .013-.766). The principal combined outcome occurred in 8 participants (23%) in SoC and in 2 (5.4%) in zSoC (OR: 0.18;95% CI .03-.946). In a logistic regression model adjusting by age, sex, comorbidity and severity the OR for the combined outcome in those in zSoC was 0.091 (95% CI: 0.007-0.913;p=0.045). LoS was shorter in zSoC (6.9 days (SD 6.1) vs 12.7 (SD 11.6);p=0.013) respectively. Time to clinical stability was significantly shorter in zSoC (5 days (SD 6.1)) compared to SoC (11.9(SD 9.1));p=0.005. No significant differences in changes in inflammatory markers were found among groups. No severe adverse events were observed during the study. Conclusion(s): Daily zinc supplementation with 240 mg of zinc acetate for 14 days during the acute phase of SARS-CoV-2 infection resulted in lower rates of severity (less death and ICU admission) and faster clinical recovery along with shorter hospital stay.

ABSTRACT

Kawasaki disease (KD) is an acute, self-limiting, and medium-sized vasculitis, which has been the commonest cause of acquired heart disease in children in developed countries. Without timely diagnosis and treatment, up-to 25% of the affected children may develop coronary artery abnormalities (CAA). Due to the lack of the specific diagnostic method, KD is mainly diagnosed according to the clinical criteria. As a result, typical KD is recognized easily, but it is a big challenge to diagnose KD patients with incomplete or atypical symptoms. The pandemic of novel coronavirus disease 2019 (COVID-19) around the world makes the diagnosis of KD even more complex. In this review, hot issues in diagnosing KD were discussed according to the 2017 guidelines for diagnosis and treatment of KD recently published by the American Heart Association (AHA), expecting to provide help for diagnosis of KD children.Copyright © 2020 by the Chinese Medical Association.

ABSTRACT

Currently, Bacille Calmette-Guerin(BCG) is still the only admitted vaccine to prevent tuberculosis around the world. The target population is infants and children, but its protective efficacy is limited. As more and more studies have shown that re-vaccination with BCG protects against tuberculosis in adults, BCG can also induce non-specific immunity against other respiratory diseases and some chronic diseases by training immunity, especially the immune effects against COVID-19. At present, the epidemic of COVID-19 has not been effectively contained, and it is worth considering whether BCG vaccine can be used as an intervention to prevent COVID-19. The WHO and China do not have a policy to support BCG revaccination, and as more and more BCG vaccines are discovered, whether selective revaccination can be carried out in some high-risk populations and whether the vaccine can be used more widely have led to intense discussions. This article reviewed the effects of specific immunity and non-specific immunity of BCG on tuberculosis and non-tuberculous diseases.

ABSTRACT

Immunotherapy plays an important role in cancer treatment by activating or suppressing the immune system. However, there are still a series of challenges to overcome regarding the delivery vehicles of immunotherapeutic agents and their effective activation at tumor sites. Meanwhile, owing to their well-hydrated environment and capability of immobilizing biological cargos, hydrogels in combination with immunotherapies provide a chance to enhance the antitumor immune response with reduced side effects. In addition, stimuli-responsiveness has been also widely applied to optimize the pharmacokinetics with an improved therapeutic outcome. In this review, we discuss the opportunities for the combination of immunotherapy and stimuli-responsive hydrogels, such as light, temperature, ultrasound and magnetically responsive hydrogels, for effective cancer treatment. Finally, we explore the potential of stimuli-responsive hydrogels as vaccine implants against cancer and Covid-19.

ABSTRACT

Detecting protective measures (e.g., masks, goggles and protective clothing) is a momentous step in the fight against COVID-19. The detection mode of unmanned devices based on Simultaneous localization and mapping (SLAM) and fusion technology is more efficient, economical and safe than the traditional manual detection. In this paper, a tightly-coupled nonlinear optimization approach is used to augment the visual feature extraction of SLAM by the gyroscope of the IMU to obtain a high-precision visual inertial system for joint position and pose estimation. Based on the VINS-Mono frame, first, an LSD algorithm based on a conditional selection strategy is proposed to extract line features efficiently. Then, we propose recovering missing point features from line features. Moreover, we propose a strategy to recover vanishing point features from line features, and add residuals to the SLAM cost function based on optimization, which optimizes point-line features in real time to promote the tracking and matching accuracy. Second, the wavelet threshold denoising method based on the 3σ criterion is used to carry out real-time online denoising for gyroscope to improve the output precision. Our WD-PL-VINS was measured on publicly available EuRoC datasets, TUM VI datasets and evaluated and validated in lab testing with a unmanned vehicle (UV) based on the NVIDIA Jetson-TX2 development board. The results show that our method's APE and RPE on MH-03-easy sequences are improved by 69.28% and 97.66%, respectively, compared with VINS-Mono. © 2001-2012 IEEE.

ABSTRACT

Overall survival (OS) is considered the standard clinical endpoint to support effectiveness claims in new drug applications globally, particularly for lethal conditions such as cancer. However, the source and reliability of OS in the setting of clinical trials have seldom been doubted and discussed. This study first raised the common issue that data integrity and reliability are doubtful when we collect OS information or other time-to-event endpoints based solely on simple follow-up records by investigators without supporting material, especially since the 2019 COVID-19 pandemic. Then, two rounds of discussions with 30 Chinese experts were held and 12 potential source scenarios of three methods for obtaining the time of death of participants, including death certificate, death record and follow-up record, were sorted out and analysed. With a comprehensive assessment of the 12 scenarios by legitimacy, data reliability, data acquisition efficiency, difficulty of data acquisition, and coverage of participants, both short-term and long-term recommended sources, overall strategies and detailed measures for improving the integrity and reliability of death date are presented. In the short term, we suggest integrated sources such as public security systems made available to drug inspection centres appropriately as soon as possible to strengthen supervision. Death certificates provided by participants' family members and detailed standard follow-up records are recommended to investigators as the two channels of mutual compensation, and the acquisition of supporting materials is encouraged as long as it is not prohibited legally. Moreover, we expect that the sharing of electronic medical records and the legal disclosure of death records in established health registries can be realized with the joint efforts of the whole industry in the long-term. The above proposed solutions are mainly based on the context of China and can also provide reference for other countries in the world. © 2022 The Authors

ABSTRACT

To explore the movement of "city-suburb" tourism flow in the post-pandemic period, this study examines the tourist flow network of the Guangdong-Hong Kong-Macao Greater Bay Area from 2018 to 2021 based on online travel data. After screening and deduplication, 4882 valid travelogues were chosen and divided into pre-pandemic data (3, 967 articles) and post-pandemic data (915 articles) using November 2019 as the dividing line. A total of 4, 461 attractions on Ctrip. com were selected to build a scenic spot database of the Guangdong-Hong Kong-Macao Greater Bay Area, including the full names, aliases (common names), and city names of scenic spots. After matching the travelogues with the scenic spots in the attraction database, it was found that 1848 attractions appeared in the travel notes, and the top 300 attractions were chosen for the generation of tourism routes according to the number of matches. After converting travel routes to a directed connectivity matrix and the following dichotomization procedure, a social network analysis (SNA) was conducted to investigate the distribution of tourism flows and preferences in the Greater Bay Area. Using the SNA software Ucinet 6.0, the network density, centrality, and relevant metrics of the structural holes and cohesive subgroups were calculated. The node characteristics and network structure were analyzed, and the distribution characteristics of attractions and tourist intention trends in the Greater Bay Area were obtained. The study results indicate the following: 1) The tourism network density of the Greater Bay Area has decreased substantially since the COVID-19 outbreak. Megacities, especially overseas cities, were more affected by the pandemic. Tourism network connectivity and aggregation effects were severely weakened, and the network structure was more scattered and fragmented. 2) After the pandemic, the "core-periphery" structure of tourism networks weakened, and the boundaries between core and periphery areas blurred. Some suburban and rural scenic spots have become new core areas and their importance in the network has been significantly enhanced. 3) After the pandemic, the connectivity and control power of traditional core urban nodes, such as Hong Kong, Zhuhai, Macao, and Guangzhou, weakened. The cohesive subgroups of scenic points show a high cohesion of Guangzhou with cities on the west side of the Pearl River Estuary, such as Foshan, Zhuhai, and Macao, before the pandemic. After the pandemic, the cohesive subgroup of rural attractions was strengthened and tourism showed a development trend of multi-point and ruralization. 5) The tourism network has changed from the three-core development mode of Guangzhou-Hong Kong-Macao before the pandemic to the "four-wheel drive" mode of Guangzhou-Macao-Shenzhen-Foshan after the outbreak with the declined linkage of the Hong Kong-Zhuhai-Macao Bridge in the network. It is believed that travel mobility restrictions and tourism drivers are two-way driving forces for the structural change in tourism in the post-pandemic period. The Greater Bay Area's tourist flow network presents a two-loop structure with two-way dynamics owing to some pandemic factors, and may gradually show a decentralized and scattered development trend. Based on network analysis, it is proposed that more efforts be made to integrate city-suburb-countryside resources in the post-pandemic period. © 2022 Editorial Committee of Tropical Geography. ABSTRACT

To explore the influencing factors of perceived stress among frontline nurses during COVID-1 9, and the role of positive psychological capital in anxiety sensitivity and stress perception, so as to provide evidence for reducing the perceived stress of frontline nurses. From December 2021 to January 2022, 475 frontline nurses from 8 hospitals in X city were investigated by general data questionnaire, positive psychological capital, anxiety sensitivity index and perceived stress scale. The results showed that there were significant differences in nurses’ perceived stress in age, working years, professional title and salary. Perceived stress was positively correlated with anxiety sensitivity, negatively correlated with positive psychological capital, and anxiety sensitivity was negatively correlated with positive psychological capital. Anxiety sensitivity can not only directly predict perceived stress, but also negatively affect perceived stress through positive psychological capital. Therefore, during the epidemic prevention and control in COVID-1 9, nurses can obtain effective stress coping strategies by strengthening their positive psychological capital, thus reducing their anxiety and perceived stress, and better providing high-quality nursing services for patients. © 2022, Editorial department of Chinese Medical Ethics. ABSTRACT

In recent years, with the frequent occurrence of viral diseases accompanied by high morbidity and mortality rates, there has been an increasing awareness of importance of antiviral drugs research. Traditional Chinese medicine contain biologically structurally diverse bioactive substances that provide important template structures for pharmaceutical research. Because of its novelty, multicomponent and multi-target characteristics, it is a valuable source for new drug development. The antiviral mechanisms of active components of traditional Chinese medicines include inhibition of viral replication, block binding of virus with receptor, directly killing virus, enhancement of the immune system and inhibition of cytokines/chemokines responses, etc. The active components of traditional Chinese medicine with antiviral active ingredients based on the mechanism of antiviral action were reviewed in this paper, in order to provide a basis for development of antiviral natural drugs to cope with the virus epidemic including the new variant of SARS-CoV-2 and other virus outbreaks that may occur in the future. © 2022 Editorial Office of Chinese Traditional and Herbal Drugs. ABSTRACT

The National Institute of Health (NIH) launches the RADx Radical research collaboratives (RADx-rad) to advance new, non-traditional approaches for COVID-19 testing. RADx-rad projects are required to adopt common data elements (CDEs) to collect data to increase data interoperability. To overcome the challenges in finding appropriate CDEs for a wide range of study variables, we create a web application - IMI-CDE to ease the burden of mapping study variables to CDEs from researchers. IMI-CDE can automatically recommend CDE candidates for a study variable based on its name and description. Together with interactive mapping interfaces, IMI-CDE allows researchers to perform variable-CDE mapping with one mouse click. In addition, the IMI-CDE application supports users with multiple roles to work collaboratively on the mapping tasks. We have piloted the IMI-CDE with RADx-rad projects. 22 researchers from 8 different projects have started to use the IMI-CDE system for variable-CDE mappings. The beta-testing evaluators reported the system is intuitive, effective, and easy to use. © 2022 IEEE.

ABSTRACT

Objective: To evaluate the transmissibility of 2019-nCoV Omicron variant under the current prevention and control strategy in Beijing, and provide evidence for the prevention and control of COVID-19. Methods: The information of 78 Omicron variant infection cases involved in clear transmission chains in Beijing during 7-25 March, 2022 were collected, the incubation period and serial interval of the disease were fitted by using Gamma and Weibull distribution. Markov Chain Monte Carlo approach was used to estimate the time-varying reproduction number (Rt). Results: The median of the incubation period (Q1, Q3) of Omicron variant infection was 4.0 (3.0, 6.0) days, and the serial interval was 3.0 (2.0, 5.0) days. The median of the serial interval (Q1, Q3) was 2.0 (1.0, 4.0) days in unvaccinated cases and 4.0 (2.0, 6.0) days in vaccinated cases (Z=-2.12, P=0.034), and 2.0 (1.5, 3.0) days in children and 4.0 (2.0, 6.0) days in adults, respectively (Z=-2.02,P=0.044), the differences were significant. The mean of Rt was estimated to be 4.98 (95%CI: 2.22-9.04) for Omicron variant in this epidemic. Conclusion: Omicron variant has stronger transmissibility compared with Delta variant. It is necessary to strengthen the routine prevention and control COVID-19, promote the vaccination and pay close attention to susceptible population, such as children.

ABSTRACT

Background: GC012F is a B cell maturation antigen (BCMA)/CD19 dual-targeting CAR-T developed on the novel FasT CAR-T platform with overnight manufacturing and designed to improve depth of response and efficacy. Data was presented at ASCO and EHA 2021 for initial 19 pts. We present updated data for study (NCT04236011;NCT04182581) with longer follow up and 9 additional pts treated (n = 28) in 3 different dose levels. Methods: From October 2019 to November 2021, 28 heavily pretreated RRMM pts (age 27-76) median of 5 prior lines (range 2-9) were treated on a single-arm, open label, multicenter Investigator Initiated Trial receiving a single infusion of GC012F. 89.3% (25/ 28) were high risk (HR- mSMART), 8 pts had EM disease, 3 had never achieved a CR including after transplant, 1 pts presented with plasma cell leukemia, 24/28 pts were refractory to last therapy, 3 pts primary refractory. 9/28 pts had received prior anti-CD38, 27/28 pts prior IMiDs. 26/28 pts were refractory to PI, 26/28 pts to IMiDs. After lymphodepletion over 2-3 days (30 mg/m2/d, 300mg/ m2/d Flu/Cy) GC012F was administered as single infusion at 3 dose levels: 1x105/kg (DL1) n = 2, 2x105/kg (DL2) n = 10 and 3x105/kg (DL3) n = 16. Results: As of Jan 26th 2022, 28 pts - median follow-up (f/ u) 6.3 mths (1.8-29.9) - had been evaluated for response. Overall response rate (ORR) in DL1 was 100% (2/2)- DL 2 -80% (8/10) DL 3 -93.8% (15/16) with 27 pts MRD negative by flow cytometry (sensitivity 10-4-10-6). 100% of MRD assessable pts (27/27) achieved MRD negativity. One patient out of 28 could not get assessed. At d28, 21/24 assessable patients were MRD negative (81.5%), 4/ 28 pts could not get d28 MRD assessment f/u due to COVID-19 restrictions however were assessed at a later timepoint. To date best response is MRD- sCR in 21/28 patients(75.0%) across all dose levels. Some pts after short f/u show responses that are still deepening. Cytokine Release Syndrome (CRS) was mostly low grade: gr 0 n = 3 (10.7%), gr 1-2 n = 23 (82.1%), gr 3 n = 2 (7.1%) - no gr 4/5 CRS and no ICANs were observed (Graded by ASBMT criteria). Median duration of CRS was 3 d (1-8 d). PK results showed no difference amongst dose levels DL1 to DL3. Overall, CAR-T median Tmax was 10 d (range 8-14 d), median peak copy number (Cmax) was 97009 (16,011-374,346) copies /μg DNA with long duration of persistence of up to d793 (data cut-off). CAR-T geometric mean AUC0-28 for DL1, DL2 and DL3 were 468863, 631540 and 581620 copies/μg DNA×day, respectively. Pts continue to be monitored for safety and efficacy including DOR. Conclusions: BCMA-CD19 dual FasT CAR-T GC012F continues to provide deep and durable responses with a favorable safety profile in additional RRMM pts across all dose levels demonstrating a very high MRD negativity rate including in pts refractory to anti-CD38, PI and IMIDs. GC012F is currently being studied in earlier lines of therapy as well as additional indications.

ABSTRACT

This paper aims to empirically examine the impact of institutional pressure on green supply chain management (GSCM) efforts and the moderating role of big data analytics capabilities (BDAC) on organizational performance. This study greatly develops a research model by integrating institutional theory, the natural resource-based view (NRBV), and dynamic capability theory to explore this relationship. This article is based on structured questionnaire data of 347 supply chain personnel. We employed structural equation modeling to verify the research hypotheses. The findings provide empirical support for institutional pressures affecting GSCM efforts and organizational performance. The results also showed that the moderating effect of BDAC positively strengthened the impact of GSCM effort on organizational performance. The findings extend and refine the existing GSCM literature, providing new insights for scholars to explore this view further. Practitioners can turn their attention to incorporating institutional pressures and advanced technologies into organizational decision-making, even in times of crisis such as Covid-19.

Objective To evaluate the specificity of three consecutive batches of 2019-nCoV nucleic acid detection kit (fluorescence PCR) manufactured by Shanghai GeneoDx Biotech Co., LTD. Methods A total of 55 common respiratory pathogens, including endemic human coronaviruses (HKU1, OC43, NL63 and 229E), severe acute respiratory syndrome conronavirus (SARS-CoV), Middle East respiratory syndrome coronavirus (MERS), seasonal influenza virus, rhinovirus, adenovirus, Klebsiella pneumonia and Mycobacterium tuberculosis, were grouped and used for verification of cross reactivity of the detection kit. According to the requirements in the Key Points of Technical Review for Registration of 2019 New Coronavirus Nucleic Acid Detection Reagents issued by Center for Medical Device Evaluation, National Medical Products Administration (NMPA), human mucoprotein, human blood, phenylephrine, oxymetazoline, sodium chloride (including those as preservatives) and other 29 interfering substances were selected to verify the anti-interference substances of this kit. Results In the verification for cross reactivity, the test results of mixed positive samples by the three consecutive batches of kit were positive, while those of negative samples were negative, indicating a coincidence rate of accuracy of 100%. The cross-reactive substances showed no effect on the test result by the kit. All the test results of positive and borderline positive samples were positive, while those of negative samples were negative, indicating a coincidence rate of accuracy of 100%. All the 29 kinds of endogenous / exogenous interfering substances showed no influence on the test results by this kit. Conclusion Hie new coronavirus 2019-nCoV nucleic acid detection kit (fluo¬rescence PCR method) manufactured by Shanghai GeneoDx Biotech Co., LTD. showed no cross-reactivity with 55 common respiratory pathogens, while showed anti-interference properties against endogenous and exogenous interfering substances such as host tissue, common respiratory pathogen and common drugs for respiratory diseases in clinic. The specificity test result of the kit met the requirements for registration of 2019 new coronavirus nucleic acid detection reagents. The test results of three consecutive batches of kits were highly stable.

ABSTRACT

As social media becomes more and more popular, fake news spreads rapidly which is more likely to cause serious consequences, especially during the COVID-19 pandemic. On the premise of meeting data privacy and security requirements, federated learning uses multi-party heterogeneous data to further promote machine learning. This paper proposes a federal learning based COVID-19 fake news detection model with deep self-attention network (FL-FNDM). We construct a deep self-attention network for fake news detection, which combines self-attention-based pretrained model BERT and deep convolutional neural network to detect fake news. Moreover, the fake news detection model is learned under the framework of horizontal federated learning, aiming at protecting users' data security and privacy. The experimental results demonstrate that the proposed model can improve the performance of fake news detection on the COVID-19 dataset, which can achieve almost the same effect of sharing data without leaking user data. © 2021 IEEE.
